Vision & Hearing Screenings
Each year our school Nurse, Dr. Wilson, will be conducting vision and hearing screenings for all students. These screenings are routine and required by state law to ensure our students are healthy and ready to learn.
If you have any questions, concerns, or would like to opt your student out, please contact Nurse Wilson at 937-325-7368 x121.
Mobile Health Clinics at CTC
We know students are better able to learn, participate, and succeed when their health needs are met. That’s why we partner with trusted healthcare providers to bring mobile health services directly to our campus.
We also know scheduling appointments, arranging transportation, and fitting healthcare into an already busy family schedule isn’t always easy. By offering access to care during the school day, we hope to make it easier for students to get the care they need so they can stay focused on learning, building skills, and getting READY for what comes next.
Asthma Inhaler or EpiPen
Students who carry an inhaler or EpiPen must complete and have a physician-completed form on file with the Health Clinic. The form can be downloaded from FinalForms. Login at SCCTC.org/FinalForms and download the Asthma Inhaler and EpiPen Self-Medication Form from the Medications area.
Medications Taken During the School Day
Students who need medications administered during the school day must have an Authorization for Prescribed Medication/Drug Treatment Form completed by a parent and licensed prescriber. The form can be downloaded from FinalForms. Login at SCCTC.org/FinalForms and download the Authorization for Prescribed Medication/Drug Treatment Form from the Medications area.

Emergency Contacts
- Parents must fill out emergency contacts on FinalForms before the beginning of the year
Medications
- Over-the-counter medications require parents to fill out (form name)
- Medications must come unopened and in the original container.
Login at SCCTC.org/FinalForms and head to the Medications area to download any required forms.
When to Stay Home
If a student is experiencing:
- Fever of 100°F or higher
- Diarrhea or vomiting in the past 24 hours
Before returning, students should be symptom-free for 24 hours without medication.
