Online Forms Now Available to be Completed

Springfield-Clark CTC has partnered with eSchoolView to simplify our back to school forms process.  The new system reduces the number of forms that you have to complete while removing the redundancy in completing general student information on the top of every form.  This new solution also allows information to be available to staff much sooner and more efficiently.

The OneView Portal is now available for parents to fill out this year’s back to school forms.  All forms must be completed online by using this portal.  If you are unable to complete the forms online at home, devices and assistance are available at the CTC to complete the forms.  The portal can be accessed through the CTC website or by following the link below.

The electronic online forms need to be completed prior to the first day of school.  Without these forms complete, students will not be able to receive their Chromebook or participate in the Career Tech lab program.

  1. Open a web browser and navigate to https://springfieldcc.esvportal.com/

  2. Click “Create A New Account” on the left side of the screen.  Fill out the necessary information and click “Register & Begin”

  3. You will then see a page allowing you to attach students to your account.  Complete the information listed under “Option 2” to pair your student with your account. To complete the registration you will need the Student Last Name, Grade Level, Date of Birth, and Student Number.  Student Numbers are located on student schedules that can be picked up at Open House. If you do not have the Student Number and you would like it before Open House, please pick it up when you get your ID picture taken or contact the school to obtain this information.

  4. Pair the student with your parent account and click “Attach”.

  5. Now click on the “Submit & View Online Forms” button.

  6. Select the “Student Demographics and EMA” form listed under the 2017-2018 Back to School Forms section.

  7. Complete all aspects of this form and click “Save & Submit to District.”  If you want to save your progress and complete the forms later, you can do so by clicking “Save.”

  8. After the form is complete and submitted, complete the Parking Form if your student will be driving a car to the CTC campus.

  9. Once all forms have been submitted to the CTC, you have completed all of the Back to School Forms for the 2017-2018 school year.
